Proposed Diabetes Personal Care Plan

Increasingly, the care of a diabetes patient is being personalised and the patient is expected to have more input and direction over the care they receive. With this in mind, the PCT has developed this proposed ‘Personal Care Plan’; the idea is that we the patient hold this document as a record of our condition, test results, medication and care. A bit like an Appraisal or Personal Review at work, ideally, you can use this to prompt concerns and questions and have a more satisfying consultation with your health care professional.

It is exciting that we can be offered an input to proposals like this, so please, click here  NS Db Care Plan V2  to open the draft document.
